  has multiple pointers to a record in table <str>.
  The logical dbkey in the index is <num>:<num>:<num>.

  Explanation:  An index contains a dbkey for  a  record  but  the
  dbkey  is  not a valid one for the relation corresponding to the
  index or the index contains multiple instances of the dbkey.

  User Action:  Recreate the index for the table.
